The two terms get used almost interchangeably in casual conversation, which causes two very different problems. Some trainees start a "systematic review" and are later asked where the pooled effect estimate and forest plot are — not realizing a systematic review doesn't have to include a meta-analysis at all. Others run a meta-analysis by pooling numbers from a handful of studies that were never systematically identified or screened, producing a pooled estimate that isn't scientifically defensible.
The two are related but distinct: one is a research method for identifying and appraising literature, and the other is a statistical technique that can — but doesn't have to — be applied within one. Understanding the difference determines both your workload and what your supervisor or reviewers will expect to see.
Why This Happens
The terms are used loosely in speech
"I'm doing a systematic review and meta-analysis" is said as a single phrase so often that the two start to feel inseparable, even though many published systematic reviews contain no meta-analysis at all — often because the included studies are too heterogeneous to pool meaningfully.
Meta-analysis looks like the "advanced" version
Because meta-analysis involves statistical pooling, it's easy to assume it's simply a more rigorous systematic review, rather than understanding it as a distinct statistical method with its own separate requirements — chiefly, that the included studies are similar enough to combine.
PRISMA and PROSPERO aren't always introduced early
Both are central to running either type of review correctly, but they're often only mentioned once a draft is already well underway, rather than at the planning stage where they matter most.
Common Mistakes We See
- Treating "systematic review" and "meta-analysis" as synonyms. A systematic review is the method of comprehensively finding and appraising evidence; a meta-analysis is one possible statistical output of that process.
- Skipping PROSPERO registration. Registering the review protocol in advance is expected by most journals and prevents duplicate effort with other ongoing reviews on the same question.
- Not following the PRISMA checklist and flow diagram. Reviewers increasingly check for both explicitly, and their absence is a common reason for immediate revision requests.
- Pooling clinically heterogeneous studies. Combining studies with meaningfully different populations, interventions, or outcomes into a single pooled estimate, without checking the I² statistic for heterogeneity first.
- Using the wrong risk-of-bias tool. Cochrane RoB2 is designed for randomized trials; ROBINS-I is designed for non-randomized studies. Using the wrong one undermines the quality assessment.
